Kelly Chew, Second Harvest\u2019s director of services says, \u201cWe recognized that food insecurity often goes hand in hand with poor health outcomes. This holistic approach allows us to tackle both hunger and health issues more effectively, ultimately creating a stronger, healthier community.&#8221;<\/p>\n\n\n\n<p>Second Harvest has developed partnerships with 61 medical partners with more than 400 medical providers across Santa Clara and San Mateo counties. We help partners incorporate food insecurity screening into standard patient intake procedures and connect individuals to resources when they screen positive. This is filling an important need for healthcare providers.&nbsp;&nbsp;<\/p>\n\n\n\n<p>\u201cAs we started gathering data about the needs of our patients, food and connections to food was one of the things that most commonly showed up,\u201d explained Mariana Becerra, enhanced care management\/community supports program supervisor at Gardner Health Services, which provides comprehensive medical and mental healthcare to patients in Santa Clara and San Mateo counties. \u201cWe started looking into ways to connect our patients to food and began working with Second Harvest. It\u2019s been really helpful.\u201d&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-expanding-access-to-nutritious-food-nbsp\">Expanding access to nutritious food&nbsp;<\/h2>\n\n\n\n<p>These food bank and healthcare partnerships reduce barriers for patients to access food assistance and also:&nbsp;&nbsp;<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>enable more patients to be connected to short- and long-term nutrition assistance programs like CalFresh and other programs;&nbsp;<\/li>\n\n\n\n<li>increase provider capacity to work food insecurity relief strategies into patient treatment plans; and&nbsp;<\/li>\n\n\n\n<li>ultimately, help prevent the onset of diet-related health conditions.&nbsp;<\/li>\n<\/ul>\n\n\n\n<div style=\"height:18px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p>\u201cOne of the most important things we focus on in our program is to help patients that have chronic conditions like diabetes and making sure we\u2019re able to help them lower their A1C,\u201d said Becerra. \u201cWe can provide so much health education but if there\u2019s no access to healthy food, then there\u2019s only so much we can do. By fiscal year 2023, that number had more than doubled to 61, including:&nbsp;<\/p>\n\n\n\n<div class=\"wp-block-columns is-layout-flex wp-container-core-columns-is-layout-9d6595d7 wp-block-columns-is-layout-flex\">\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\" style=\"flex-basis:33.33%\">\n<ul class=\"wp-block-list\">\n<li>Kaiser Permanente&nbsp;<\/li>\n\n\n\n<li>Santa Clara Valley Medical Center \u2013 Hospital and Clinics&nbsp;<\/li>\n\n\n\n<li>San Mateo Medical Center&nbsp;<\/li>\n\n\n\n<li>Stanford Health Care&nbsp;<\/li>\n\n\n\n<li>Stanford Medicine Children\u2019s Health<\/li>\n<\/ul>\n<\/div>\n\n\n\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\" style=\"flex-basis:66.66%\">\n<div style=\"min-height: 291px;\" id=\"datawrapper-vis-ilBEp\"><script type=\"text\/javascript\" defer src=\"https:\/\/datawrapper.dwcdn.net\/ilBEp\/embed.js\" charset=\"utf-8\" data-target=\"#datawrapper-vis-ilBEp\"><\/script><noscript><img decoding=\"async\" src=\"https:\/\/datawrapper.dwcdn.net\/ilBEp\/full.png\" alt=\"Displays the total number of medical partners by fiscal year. \" \/><\/noscript><\/div>\n<\/div>\n<\/div>\n\n\n\n<p>\u201cThis [access to food] is so critical,\u201d shared a provider. \u201cI often hear myself telling patients, \u2018Take this with the largest meal of the day,\u2019 and meanwhile they are thinking about their lack of food. It makes all the difference.\u201d&nbsp;&nbsp;<\/p>\n\n\n\n<p>\u201cThank you so much for asking me,\u201d said a patient. \u201cI didn\u2019t know how or think to tell you I didn\u2019t have enough. I am so grateful.\u201d&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-helping-thousands-of-local-patients-nbsp\">Helping thousands of local patients&nbsp;<\/h2>\n\n\n\n<div class=\"wp-block-columns is-layout-flex wp-container-core-columns-is-layout-9d6595d7 wp-block-columns-is-layout-flex\">\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\" style=\"flex-basis:66.66%\">\n<div style=\"min-height: 292px;\" id=\"datawrapper-vis-Ge04U\"><script type=\"text\/javascript\" defer src=\"https:\/\/datawrapper.dwcdn.net\/Ge04U\/embed.js\" charset=\"utf-8\" data-target=\"#datawrapper-vis-Ge04U\"><\/script><noscript><img decoding=\"async\" src=\"https:\/\/datawrapper.dwcdn.net\/Ge04U\/full.png\" alt=\"Displays the total number of food referrals by fiscal year. \" \/><\/noscript><\/div>\n<\/div>\n\n\n\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\" style=\"flex-basis:33.33%\">\n<p>Second Harvest\u2019s medical partnerships have identified and helped thousands of patients who may be food insecure. In fiscal year 2021, providers referred 765 patients to Second Harvest, and, by fiscal year 2023, that number had grown to 2,174 patients.<\/p>\n<\/div>\n<\/div>\n\n\n\n<p>\u201cI was at first very uncomfortable but then the doctor explained why it is important to get healthy food for medical reasons and they really made sense to me,\u201d said another patient. \u201cI was not uncomfortable anymore.\u201d&nbsp;<\/p>\n\n\n\n<p>A provider shared that, \u201csomeone screened positive for depression but when I talked to them, they were just hungry. If I wouldn\u2019t have learned to ask about food, I would have just sent them to the therapist.\u201d&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-expanding-reach-to-serve-more-neighbors-needing-food-assistance-nbsp\">Expanding reach to serve more neighbors needing food assistance&nbsp;<\/h2>\n\n\n\n<p>The vast majority of patients served through this program were households with children or seniors, enabling us to reach some of the area\u2019s most vulnerable populations by partnering with medical providers.&nbsp;<\/p>\n\n\n\n<table id=\"tablepress-66\" class=\"tablepress tablepress-id-66\">\n<thead>\n<tr class=\"row-1\">\n\t<th class=\"column-1\">FY2023<\/th><th class=\"column-2\">Total Households Assisted with Food Referral<\/th>\n<\/tr>\n<\/thead>\n<tbody class=\"row-striping row-hover\">\n<tr class=\"row-2\">\n\t<td class=\"column-1\"><strong>Households with seniors<\/strong><\/td><td class=\"column-2\">473 (27%)<\/td>\n<\/tr>\n<tr class=\"row-3\">\n\t<td class=\"column-1\"><strong>Households with children (18 under)<\/strong><\/td><td class=\"column-2\">757 (43%)<\/td>\n<\/tr>\n<tr class=\"row-4\">\n\t<td class=\"column-1\"><strong>Average household size<\/strong><\/td><td class=\"column-2\">2.8<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<!-- #tablepress-66 from cache -->\n\n\n\n<p>\u201cIt\u2019s been a really useful tool to have, that easy connection into Second Harvest,\u201d said Becerra. \u201cTo be able to submit the referrals and then help the patients. It\u2019s been very helpful.\u201d&nbsp;<\/p>\n\n\n\n<p>\u201cOur medical partnership program is enhancing community health by fostering collaboration between healthcare providers and community members.\u201d said Chew. \u201cAccess to healthy food is a key part of our strategy, helping to prevent and manage chronic diseases.
