La Wellness Pantry \u00e8 orientata verso persone a basso reddito che hanno una o pi\u00f9 di cinque condizioni di salute: prediabete, diabete gestazionale, diabete, ipertensione o colesterolo alto. <strong>Questo programma \u00e8 stato creato per aiutare a migliorare le condizioni mediche di un paziente insieme ai servizi medici forniti dal proprio medico.<\/strong> \u00c8 stato dimostrato che l&#039;accesso a cibi pi\u00f9 sani e nutrienti pu\u00f2 portare a migliorare la salute dei pazienti, abbassare i loro costi medici (meno farmaci, tempo trascorso in ospedale) e consentire loro di essere pi\u00f9 attivi fisicamente.<\/p>\n<p><strong>La clinica serve circa 7000 pazienti adulti. Una stima approssimativa di quante persone potrebbero cercare la Wellness Pantry \u00e8 di circa 800 - 1.750 persone.<\/strong> Le proiezioni di insicurezza alimentare presso la clinica rispondono anche alle esigenze alimentari di tutti i pazienti. Quando un paziente viene sottoposto a screening dal proprio fornitore medico, e viene stabilito che non ha nessuna delle cinque condizioni mediche predeterminate, ricever\u00e0 un rinvio a un altro ottimo programma alimentare Second Harvest con cibo sano vicino alla propria residenza.<\/p>\n<p>Chiunque soddisfi i criteri specificati porta il modulo di riferimento che ha ricevuto dalla Fair Oaks Health Clinic all&#039;Esercito della salvezza.
